Dementia & Alzheimer's

It takes a qualified and professional team to understand and provide memory care at home. Dementia and Alzheimer's have stages with corresponding cognitive loss and residual capabilities. Our team is able to identify how to best provide help and address instructions for your loved one. We are able to arrange for physical and cognitive re-education therapies, and we train our staff to look for signs of agitation, dehydration, lethargy, decreasing strength, decreasing range of motion, and spousal stress - as these are some of the major issues that degrade the quality of life of an elderly person suffering from cognitive loss. We provide screenings and intervention in a periodic manner and are able to monitor declines and communicate with your family and physicians for the best solutions for your loved one.
Fall Safety
If your loved one has experienced an accidental fall, he or she is at high risk for future falls, injury, and health complications. Most fall injuries occur in the bathroom or kitchen. An in-home aide can help with kitchen and bathroom tasks so to reduce the risk and give your loved one more confidence. Our aides will also help with getting up and down from furniture, keeping the home clear of tripping hazards, and performing tasks that have become too dangerous for our senior client. If the family agrees, we can also arrange for in-home physical therapy to assess for rehab potential and develop an in-home exercise plan. An occupational therapist can devise strategies that make the home and equipment work better for our client. Sometimes, falls can be related to medical conditions. A skilled nurse can monitor medication adherence, check for medication interactions, perform head to toe assessments, etc. A coordinated approach can mitigate or stop the progression of any underlying problems so your loved one is free to enjoy living in his or her own home, long term.
Health Support
Seniors who develop a need for assisted living often experience an accelerated health decline. Let's give seniors the support they need to maintain health and comfort at an optimal level while also using skilled care to interrupt any progressing problems. In home aides can help with meal preparation for optimal nutrition, medication reminders to ensure adherence, activity assistance to keep seniors active, transportation assistance to make sure client's stay on track with their doctors, and much more. Dietitians can further improve the nutrition plan. A nurse may be necessary to set up pill boxes weekly, as nurses are the correct professionals for taking medicine out of their original containers and arranging them in other containers. Nurses may also be needed to help with more involved medication administration such as injections. At the same time, nurses can perform exams to check for disease processes, perform medication reviews to check for symptoms or interactions, etc. Physical therapists can design the lowest-effort / highest-return home exercise plans to prevent general strength decline. All skilled services are coordinated with your loved one's doctor.
Transportation
Should your loved one be driving or going in and out of businesses unassisted? It's a question all families wrestle with eventually. Our aides can provide transportation assistance or assistance in the community to make it safer for our clients to go about their business. Our aides can also run errands for clients. However, Help to Seniors also wants to ask, why has running errands become more of a problem? Is diabetic neuropathy of the feet setting in, so the loved one can't feel the pedals? A nursing assessment can detect this problem and begin a treatment program. Has a senior lost range of motion in the neck so he or she can't look around, developing slow reflexes, or losing the leg strength to get in and out of cars alone? A physical and/or occupational therapy approach can determine specific deficits and develop plans to maximize health and safety. All skilled services are arranged in communication with and with orders of your loved one's physician.

If your loved one was living in a an assisted living facility - there would be routine oversight of medical, physical, and functional declines that correspond to these chronic conditions. If declines were identified, there would be care plan and services such as nursing or therapies ordered by the doctor to address those declines. We bring that same level of care to you at your home. We provide routine monitoring of your loved one's physical and medical health, and we communicate with their doctors. We can arrange for nursing or rehabilitative services if ordered.
